This story is told in a mixture of letters and reminiscences of the main players and this did create a bit of confusion as to who was 'speaking' but as there was little overlap in terms of story, this was not too bad.

The story itself is fairly unusual for Doctor Who as it was fairly close to being a proper horror story, with an unusual sanatorium offering fresh air cures for consumption (TB). As Rory points out, this puts the doctor in charge a good hundred years ahead of his time; in 1783 the consumptive was generally condemned to death. But Dr Bloom's sanatorium had an unusual success rate.

And why were the patients left out on the beach in all weathers? There's fresh and there's icy!

With the TARDIS on the run from an attack and the Doctor's mind under seige from a powerful psychic attack, the three Time Travellers have to solve the mystery while having lost some of their memories after the TARDIS crashed!
